You may change ownership by completing an Absolute
Assignment form in full.
When completing the form, keep in mind that if there are multiple current
or new policyowners, all must sign.
Whenever one or more individuals are signing for a trust, corporation or other
institution, be sure they each include their title with their signature. If
the new policyowner is a trust, the trustees must complete a Trust
Certification form. If the new policyowner is a corporation or business, the
authorized officers of that business must complete a Business Entity Ownership
Certification.

may also wish to change the beneficiary of the policy as well – please
