Q: Who is eligible to participate in the program?
A: Any group who currently has their NJTDB plan with Security Mutual Life. (If you do not currently have your NJ Temporary Disability Benefits with Security Mutual Life and would like to sign up for this coverage, please see the New Jersey Temporary Disability Benefits application below.)
Q: I am an employer who is interested in taking
advantage of bundling the NJTDB and NJFLB plans with Security Mutual Life. What do I need to do?
A: It’s easy! Simply complete the NJFLB application and Security Mutual Life consent forms below, and submit both to Security Mutual Life Insurance Company of New York, P. O. Box 1625, Binghamton, New York 13902-1625.

You must obtain at least 51% of your employees’ signatures to move from the state fund into our private plan.
Q: If I am an employer with more than 120 employees, what do I need to do?
A: Your NJTDB rate will be based on the claims experience for your group, which Security Mutual Life will obtain for you. Just email Marcie Calhoun at MCalhoun@smlny.com to request a quote.
Q: What kind of savings will I get by moving my plan from the state plan to Security Mutual Life?
A: You will receive a discount of .05% off your NJTDB rate when you move your NJFLB plan to Security Mutual Life. For example, if your NJTDB rate is .50% of taxable wage, it will be reduced to .45% of taxable wage. The discount will be effective on the next quarterly billing cycle (January 1, April 1, July 1, or October 1) following the acceptance of the enclosed New Jersey Family Leave Insurance Benefits application.
Q: How is the NJFLB plan billed?
A: Security Mutual Life will bill you in the same manner that we currently bill your NJTDB plan. Restaurants will receive their bill quarterly in arrears in conjunction with the NJTDB billing cycle.
Q: Is there a certain effective date required for moving my plan from the state fund to Security Mutual Life?
A: Security Mutual Life accepts applications for effective dates on a quarterly calendar basis: January 1, April 1, July 1, or October 1.

The expected benefit ratio for each policy is 65%. This ratio is the portion of future premiums that the company expects to return as benefits, when averaged over all people within the policy.
